Privacy Policy for Patients of Professor J. Zieliński Medical Centre
This document outlines the rules for processing personal data and the use of cookies by the website available at the address https://www.cmzielinski.pl (hereinafter: the „Service”). The purpose of this policy is to provide the user with information regarding the personal data controller, the type of data collected, the purposes of their use, and the principles of their protection.
§1. General Information
The personal data administrator of users is Centrum Medyczne Prof. J. Zieliński Sp. z o.o., with its registered office in Gdańsk at ul. Joachima Lelewela 6 lok.3.

§2. Purposes, legal bases and scope of data processing
User personal data is processed for the following purposes:
- User contact – response to the submitted query (legal basis: Article 6(1)(f) of the GDPR – legitimate interest of the controller).
- Cookie handling – ensuring the correct functioning of the Service (legal basis: Article 6(1)(f) GDPR).
- Analyse statistics – with the user’s consent (Art. 6(1)(a) GDPR).
- Marketing activities – with the user’s consent (Art. 6(1)(a) GDPR).
- Social media profile management – informational and marketing activities within the legitimate interests of the controller (Art. 6(1)(f) GDPR).
Scope of data processed:
- data provided voluntarily in an e-mail or contact form (e.g. name, e-mail address);
- IP address and other technical data automatically obtained as a result of using the Service;
- data shared on social media (e.g. Facebook) in connection with interaction with the administrator's profile;
- data obtained via third-party API partners.
§3. Recipients of the data and transfer of data outside the EEA
Personal data may be transferred to third parties solely to the extent necessary to fulfil the purposes mentioned in this policy, including:
• entities processing marketing and analytical cookies (with user consent),
• an analytics service provider (e.g. Google Analytics),
• partners such as Google Inc., Facebook Inc. – in accordance with their own privacy policies.
Data may be transferred outside the European Economic Area (EEA) exclusively in cases where the external service provider (e.g. Google, Facebook) is based outside the EEA and ensures an appropriate level of protection in accordance with GDPR.
§4. Data retention period
User personal data will be stored for no longer than is necessary for the purposes of processing:
• in the case of contact details – until consent is withdrawn or an objection is raised;
• data automatically collected (e.g. cookies) – according to the period saved in the user's device or until deleted;
• marketing and analytical data – a maximum of 3 years from last contact or activity.
§5. Rights of data subjects
The user is entitled to the right:
• access to its data;
• for rectification, erasure or restriction of processing;
• data transfer;
• object to the processing of data;
• withdraw consent at any time;
• lodging a complaint with the President of the Personal Data Protection Office.
An appeal against the actions of the Administrator may be lodged with the President of the Personal Data Protection Office, ul. Moniuszki 1A, 00-014 Warsaw.

§6. Information about cookies
